Consent Forms

Prior to your first appointment, we require a consent form to be filled out in its entirety. This ensures that I am fully aware of any contraindications that could impact your treatment and that I select the best products for you. You are more than welcome to fill out the form upon your arrival at the facial suite if you choose not to fill it out prior. In that case please plan to arrive 5 minutes prior to your appointment.
